DESCRIPTION:Register Here\nHigh Medical Complexity patients are individuals who survive a life-threatening illness or injury with or without multiple\, co-morbid conditions who are debilitated and dependent on intense medical care for a longer period of time. Critical illness recovery care follows acute intensive care and is provided using a multidisciplinary\, system/organ preservation and conservation treatment approach. Referral timing is critical when navigating patients through the continuum of care and the transition to the next level of care.
