DESCRIPTION:Register here!\n  \nJoin us for a presentation on Everyday Empowerment: Community Coaching for Successful Transitions.\n1 hr RN and CCM\n \nAs the pace of population aging accelerates\, the nursing home sector faces an access to care crisis. Limited admissions and downsizing have left many older adults without the necessary care and placement\, while current residents risk displacement (American Health Care Association\, 2023). This issue is compounded by the increasing demand for long-term care services. One solution is the federal Money Follows the Person (MFP) program\, which helps states rebalance their long-term care systems by transitioning individuals with long-term illnesses or disabilities from institutions back into the community. Successful and sustained transitions depend on factors such as planning\, coordination\, housing availability\, and access to home and community-based services (Colello\, 2021). However\, a significant gap identified is the lack of resources for integrating into everyday community life. Individuals often struggle with daily tasks previously managed in institutional settings\, such as getting a haircut\, finding a church\, or mailing a letter. The Community Coaching service line pilot program seeks to bridge this gap. The Community Coach is a role designed to augment services delivered by home and community-based case managers. Together\, the Community Coach and client choose needed content through the use of a topical menu and road map for a successful transition. This program focuses on enhancing life skills and fostering community connections\, which are crucial for successful reintegration. \nThis presentation will detail the need for community support\, the design of the service line\, and the pilot’s implementation. Preliminary results indicate that participants in the Community Coaching program experience smoother transitions and higher satisfaction levels. By addressing these gaps\, the program not only improves individual outcomes but also offers insights into how similar initiatives can be scaled and implemented nationwide.
