DESCRIPTION:REGISTER HERE\n  \nHosted by Hudson Valley CMSA \nObjectives: \n\nReview circumcision’s roots in American medicine from 19th century Victorian times to the present\, and the procedure’s ongoing promotion despite it being considered “non-therapeutic” by U.S. and international medical organizations.\nDescribe long term psychological and sexual consequences of amputating the male foreskin\, including personal accounts from men and their female partners.\nDiscuss the ethical dilemmas that confront medical and nursing professionals asked to participate in non-therapeutic genital surgeries to which the patient cannot consent.\n\nSpeaker: Georganne Chapin served for 25 years as CEO of Hudson Health Plan\, a nonprofit Medicaid managed care insurer in New York’s Hudson Valley; founded the Hudson Center for Health Equity and Quality—a tech company she still leads—and in 2008\, co-founded Intact America\, an organization dedicated to ending the unethical genital cutting of all children. In 2024\, she published This Penis Business: A Social Activist’s Memoir\, winning a Book Excellence Award in the health category. In the book\, Chapin describes her unusual upbringing and early awareness of circumcision due to a serious complication suffered by her younger brother. Chapin holds a BA in anthropology from Barnard College\, an MA in sociomedical sciences from Columbia University\, and a JD from Pace University School of Law. She also has taught Bioethics at Pace law school and Dominican College School of Nursing.

Limited admissions and downsizing have left many older adults without the necessary care and placement\, while current residents risk displacement (American Health Care Association\, 2023). This issue is compounded by the increasing demand for long-term care services. One solution is the federal Money Follows the Person (MFP) program\, which helps states rebalance their long-term care systems by transitioning individuals with long-term illnesses or disabilities from institutions back into the community. Successful and sustained transitions depend on factors such as planning\, coordination\, housing availability\, and access to home and community-based services (Colello\, 2021). However\, a significant gap identified is the lack of resources for integrating into everyday community life. Individuals often struggle with daily tasks previously managed in institutional settings\, such as getting a haircut\, finding a church\, or mailing a letter. The Community Coaching service line pilot program seeks to bridge this gap. The Community Coach is a role designed to augment services delivered by home and community-based case managers. Together\, the Community Coach and client choose needed content through the use of a topical menu and road map for a successful transition. This program focuses on enhancing life skills and fostering community connections\, which are crucial for successful reintegration. \nThis presentation will detail the need for community support\, the design of the service line\, and the pilot’s implementation. Preliminary results indicate that participants in the Community Coaching program experience smoother transitions and higher satisfaction levels. By addressing these gaps\, the program not only improves individual outcomes but also offers insights into how similar initiatives can be scaled and implemented nationwide. DESCRIPTION:Register here!\n  \nJoin us for a presentation on From Nursing to Case Management: Adapting a Proven Scale to Understand and Improve the Case Manager Experience.\n1 hr RN and CCM\n \nThe Practice Environment Scale of the Nursing Work Index (PES-NWI) has been widely used for more than two decades to study the nursing work environment. Professional case managers specialize in care coordination to meet the clinical and financial goals of medically and socially complex patient populations across the continuum of care. Despite its importance\, there is limited research on the case management practice environment\, underscoring the need for a valid and reliable instrument to assess this concept. This study aims to adapt the PES-NWI to the context of hospital case management. Following expert panel feedback\, the instrument was adapted for use in hospital case management (Practice Environment Scale for Hospital Case Management). A convenience sample was recruited via social media and professional networks\, yielding 160 hospital case managers working across the United States. Psychometric evaluation of the adapted instrument included assessments of validity (content validity via expert panel review\, known-groups validity\, and factor analysis) and reliability (Cronbach’s alpha and item-total correlations). Hospital case management participants worked across 34 states and were mostly female (92%) from the disciplines of nursing (79%) and social work (21%). Nearly 60% of respondents held a national case management certification\, and 40% had worked in case management for over 10 years. Factor analysis yielded a four-factor solution with clean item loadings: Supportive and Participatory Leadership\, Staffing and Resource Adequacy\, Opportunities for Professional Development\, and Fostering Teamwork and Collaboration. The factor structure differs from the original instrument; however\, psychometric testing demonstrates strong reliability (overall Cronbach’s alpha of 0.93; item-to-total correlations from 0.37 to 0.80) and validity. The Practice Environment Scale for Hospital Case Management is the first instrument to assess case managers’ practice environments in the hospital setting\, which provides a foundation for future research. Prior to joining Healthfirst he served a variety of roles including as chief medical officer of a multistate partial program and regional medical director of an eating disorder treatment provider. He is a distinguished fellow of the American Psychiatric Association and the American Academy of Child and Adolescent Psychiatry. He is co-chair of the AACAP’s Coding and Reimbursement committee and the advisor to the American Medical Association’s Relative Value Scale Utilization Committee (RUC).\nSession Summary:\n1.
