As healthcare continues to evolve\, MVD case managers must be equipped with the knowledge\, leadership\, and collaborative strategies needed to address emerging clinical challenges\, navigate complex transitions\, leverage innovative technologies\, and improve patient outcomes. \nMission Ready: Advancing Case Management for Military and Veteran Populations is a comprehensive educational experience designed specifically for case managers serving both active military service members\, Veterans\, and their families. Through eight expert-led sessions\, participants will explore evidence-based practices and innovative approaches to caring for medically complex populations\, strengthening transitions of care\, integrating palliative and specialty rehabilitation services\, addressing emerging clinical risks\, leveraging artificial intelligence responsibly\, and enhancing interdisciplinary collaboration across healthcare systems. \nAlthough many sessions highlight military and Veteran-specific programs\, the principles of coordinated care\, patient advocacy\, ethical decision-making\, and interdisciplinary practice are broadly applicable to case managers across diverse healthcare settings.
