With the rise in value-based payment models and the ongoing transformation of eldercare delivery\, I-SNPs are becoming a strategic pathway to deliver coordinated\, cost-effective\, and person-centered care. \nThis session provides an exploration of I-SNPs from conceptual foundation to practical implementation.
