DESCRIPTION:REGISTER HERE\n  \nHosted by Hudson Valley CMSA \nObjectives: \n\nReview circumcision’s roots in American medicine from 19th century Victorian times to the present\, and the procedure’s ongoing promotion despite it being considered “non-therapeutic” by U.S. and international medical organizations.\nDescribe long term psychological and sexual consequences of amputating the male foreskin\, including personal accounts from men and their female partners.\nDiscuss the ethical dilemmas that confront medical and nursing professionals asked to participate in non-therapeutic genital surgeries to which the patient cannot consent.\n\nSpeaker: Georganne Chapin served for 25 years as CEO of Hudson Health Plan\, a nonprofit Medicaid managed care insurer in New York’s Hudson Valley; founded the Hudson Center for Health Equity and Quality—a tech company she still leads—and in 2008\, co-founded Intact America\, an organization dedicated to ending the unethical genital cutting of all children. In 2024\, she published This Penis Business: A Social Activist’s Memoir\, winning a Book Excellence Award in the health category. In the book\, Chapin describes her unusual upbringing and early awareness of circumcision due to a serious complication suffered by her younger brother. Chapin holds a BA in anthropology from Barnard College\, an MA in sociomedical sciences from Columbia University\, and a JD from Pace University School of Law. She also has taught Bioethics at Pace law school and Dominican College School of Nursing.

DESCRIPTION:Register here!\n  \nJoin us for a presentation on Everyday Empowerment: Community Coaching for Successful Transitions.\n1 hr RN and CCM\n \nAs the pace of population aging accelerates\, the nursing home sector faces an access to care crisis. Limited admissions and downsizing have left many older adults without the necessary care and placement\, while current residents risk displacement (American Health Care Association\, 2023). This issue is compounded by the increasing demand for long-term care services. One solution is the federal Money Follows the Person (MFP) program\, which helps states rebalance their long-term care systems by transitioning individuals with long-term illnesses or disabilities from institutions back into the community. Successful and sustained transitions depend on factors such as planning\, coordination\, housing availability\, and access to home and community-based services (Colello\, 2021). However\, a significant gap identified is the lack of resources for integrating into everyday community life. Individuals often struggle with daily tasks previously managed in institutional settings\, such as getting a haircut\, finding a church\, or mailing a letter. The Community Coaching service line pilot program seeks to bridge this gap. The Community Coach is a role designed to augment services delivered by home and community-based case managers. Together\, the Community Coach and client choose needed content through the use of a topical menu and road map for a successful transition. This program focuses on enhancing life skills and fostering community connections\, which are crucial for successful reintegration. \nThis presentation will detail the need for community support\, the design of the service line\, and the pilot’s implementation. Preliminary results indicate that participants in the Community Coaching program experience smoother transitions and higher satisfaction levels. By addressing these gaps\, the program not only improves individual outcomes but also offers insights into how similar initiatives can be scaled and implemented nationwide. The long-term benefits of such support systems could lead to more sustainable and person-centered care models\, ultimately influencing future policy and practice in both the long-term care sector and community-based case management. \nObjectives: \n\nSummarize the current challenges faced by the nursing home sector due to the rapid pace of population aging\, including limited admissions\, downsizing\, and the resulting displacement risks for older adults.\nDescribe the goals\, mechanisms\, and successes of the MFP program in transitioning individuals from institutional settings to community living\, emphasizing the importance of planning\, coordination\, and access to home and community-based services.\nDevelop a similar Community Coaching program by leveraging the design\, implementation strategies\, and preliminary outcomes of the pilot program\, enabling other organizations to address gaps in community integration and enhance successful transitions for individuals moving from institutional care to community living.\n\nPresenters: \n\nSarah Cukale-Matos\, DNP\, RN\nKarissa Maddox\, MSN\, RN-BSN\, CMC
