SUMMARY:CMSA National (1CE) - Improving Hypertension Control in the Black and African American Population
DESCRIPTION:Registration & Information here! \nImproving Hypertension Control in the Black and African American Population\nHypertension remains a leading risk factor for cardiovascular and renal disease\, with well-documented disparities in both prevalence and control across racial groups. Our Population Health team conducted an in-depth analysis of hypertension control outcomes\, stratified by race\, to better understand inequities within our patient population. This analysis revealed that Black and African American patients were 7% less likely to have controlled hypertension compared to other racial groups. This finding is particularly concerning given that Black and African American individuals are approximately 30% more likely to develop hypertension\, underscoring a critical and persistent health inequity. \nUncontrolled hypertension significantly increases the risk of adverse health outcomes\, including myocardial infarction\, stroke\, heart failure\, dementia\, and chronic kidney disease—conditions that substantially diminish quality of life and contribute to increased healthcare utilization and costs. Recognizing the disproportionate burden borne by Black and African American patients\, our team prioritized targeted interventions aimed at improving hypertension control within this population. \nGrounded in evidence demonstrating that effective blood pressure management can prevent or delay the onset of chronic disease\, we implemented focused outreach and care strategies to support improved clinical outcomes. By addressing both short-term blood pressure control and long-term cardiovascular risk\, this work represents a critical step toward reducing health disparities and advancing equitable health outcomes for Black and African American patients. \nObjectives: \n\nDescribe and demonstrate the process used in the development of a health equity hypertension patient centered education program.\nIdentify the evidence-based practices established to improve the patient engagement in managing their hypertension.\nDiscuss and evaluate the outcome measures used to determine the success of the educational provided to this population.\n\nFee: $20 \n1 hour RN\, SW\, and CCM credits. \nPresenters:\n- Carissa Huff\, LPN\, MSA\, Quality Project Manager II\n- Maria Tout\, MHSA\, Health Equity Specialist\nTrinity Health IHA Medical Group

Lisa currently serves as Past President for the CMSA-Detroit Chapter\, on the CMSA Board of Directors and Editorial Board\, CMSA Grants Taskforce\, and is Co-Chair of the Epic Care Management Advisory Board. \nTiffany Ferguson\, CEO of Phoenix Medical Management\, Inc. Tiffany is CEO of Phoenix Medical Management\, Inc. and an adjunct professor in the Department of Social Work at Northern Arizona University. She serves on the American College of Physician Advisors (ACPA) UM OPS Committee and is co-author of The Hospital Guide to Contemporary Case Management (HCPro).
