Case Management, a Career, Not Just a Job

A panel of three career case managers will discuss their decades in CM and relate that information to the subjects of professionalism and career growth. We will provide a look back at the growth and development of case management, a review of where we are now and their thoughts of the future as case management grows, develops, evolves and adapts to changing times. We will discuss flexibility and agility and the need to be proactive as we support ourselves and our clients in a changing world. Case managers as leaders and facilitators not only for our individual clients but in terms of population health and public policy. They will introduce the audience to a compilation of the lives and opinions of dozens of case managers leaders as reflected in the book, "Second Acts: Reflections from Nurses to Inspire the Next Generation of Nurses." The speakers are contributors to the book.

Entrepreneurship: The Less-traveled Road in Case Management

As Case Managers we have many options of practicing settings in which we can impact patient outcomes and facilitate better quality of life for the populations we serve. Becoming an independent case manager in the private sector is one such way but a road less traveled by case managers. In one case manager survey, only 4 percent of respondents chose "independent/private practice" as their primary work/practice setting. The purpose of this presentation is to introduce case managers to this option by sharing my story of how I got to this place and what I have learned on this journey. The session is designed as an interactive process between those with an interest in and those who have pursued this career direction as we all strive to make a difference in health care delivery. We will discuss business plans and business development built on a foundation of integrity and fidelity as key factors in growing successful business. It is about obstacles and opportunities and reflections on our career path. "Two roads diverged in a wood, and I - I took the one less traveled by, And that has made all the difference".

Improving Processes in Reducing Readmissions

Over two year, hospitals admissions were dominated with patients presenting a primary diagnosis of COVID-19 pneumonia with hypoxia. These patients had an increased risk of readmissions. During today’s session, you will hear how one organization implemented new interventions to decrease return visits by adding closer follow-up upon discharge. They were able to provide earlier follow-up to identify transitions that were not effective and provide a level of care to provide necessary resources.

This organization will share how they were able to support the primary care time while innovatively identifying ways to support patients and reduce readmissions. You will also learn how you can identify processes improvements in your organization and create the data for measuring your outcomes. These helpful tips can help your organization demonstrate process improvement, savings, and justification for case management services.
