Clostridioides difficile: Reducing Mortality and Morbidity with Effective Care Coordination and Transitions 

The most common cause of diarrhea occurring in a health care setting is Clostridioides difficile (C. diff) and is also the cause of antibiotic-associated colitis. An estimated 500,000 cases occur during hospitalization, costing $6.3 million. Thirty percent of those with an initial infection will have a recurrence, and 40% will have a third recurrence. Effective and efficient care transitions and coordination are the solutions to reducing the incidence and adverse events. The infection results from a disruption in the microbial flora of the gastrointestinal tract, mainly after antibiotic use or other medications such as PPIs. As a result, infected individuals are colonized and shed the spores into the environment, exposing others—goals of treatment focus on reducing the exposure and individual susceptibility. 

Case Managers are the professionals prepared to address prevention, transitions, care coordination, social determinants, and inequities. Case managers are found in every health care setting, and by way of their training, they can and do address all aspects of an individual's care journey. When equipped with the latest evidence and best practices, case managers can assimilate knowledge and skills to the patient, support system, and care delivery system resulting in improved health outcomes and quality of life.

  1. Explore the incidence and impact of C. diff infection.  
  2. Define the recent changes to clinical management as recommended by industry guidelines.  
  3. Demonstrate strategies to reduce recurrence, hospital admissions, and poor quality of life.

      Presenter: 

      Rebecca Perez, MSN, RN, CCM 

      Rebecca is an experienced RN with a master’s degree in Nursing, is a Certified Case Manager, and a member of the Gamma Omega Chapter of Sigma Theta Tau International Nursing Honor Society. She is the author of numerous professional articles, a co-author of CMSA’s Integrated Case Management: A Manual for Case Managers by Case Managers, developer of the Integrated Case Management Training Program, and Master Trainer. She recently joined Parthenon Management Group as the Senior Manager of Education and Strategic Partnerships.
