For our inaugural case study, we partnered with Sandra Stein, MSN ,RN, CCM, IQCI  to produce UCI Health Discharge Standardization: A Multidisciplinary Approach. This work offers a deep look into improving discharge coordination through a multidisciplinary approach, showcasing how structured communication and shared planning tools can enhance patient flow and care team collaboration, providing actionable insights and reflective commentary that illustrate the essential role case managers play in improving outcomes.
