This paper emphasizes the importance of case management and professional case managers (PCMs) in improving care coordination and patient outcomes within primary care settings, in line with the CMS Making Care Primary (MCP) Model. It outlines effective case management practices, PCMs' roles in managing chronic conditions and social determinants of health, and the application of CMSA's standards of practice-all in primary care settings.
The paper aims to inform CMSA members, PCMs, and healthcare professionals about the MCP Model and the benefits of case management, providing evidence-based strategies for implementation.
