Organ transplantation has evolved from an experimental procedure into a life-saving intervention that offers renewed hope to thousands of patients worldwide. For professional case managers, understanding the historical progression, current landscape, and future advancements in transplantation is essential for providing informed, patient-centered care.

This presentation will explore pivotal milestones in transplantation, beginning with the first successful kidney transplant in 1954 to the pioneering liver transplant in 1967. By examining the past, present, and future of transplantation, case managers will be equipped with the knowledge needed to advocate for best practices, anticipate future challenges, and contribute to the ongoing evolution of comprehensive, transplant patient care.

Objectives:

  • Identify the critical role of case managers in the transplant process, from patient evaluation and education to long-term post-transplant care.
  • Explain the case manager’s role in coordinating multidisciplinary care for transplant patients, ensuring seamless communication between providers, patients, and caregivers throughout the transplant continuum.
  • Discuss strategies to address barriers to transplant access, including socioeconomic disparities, health literacy challenges, and psychosocial considerations.
  • Discuss best practices in case management to optimize post-transplant care, including medication adherence, complication monitoring, and long-term patient support to enhance graft survival and quality of life.
