Effective communication, screening, and assessment are critical to successful case management, yet many professionals receive limited training in these skills. As behavioral health needs continue to grow, case managers are increasingly navigating complex conversations with patients, families, and care teams.
This session introduces Behavioral Health Conversation Maps, a seven-phase framework that supports care coordination for individuals with complex chronic and behavioral health conditions. Participants will gain practical tools to strengthen engagement, assessment, communication, and ethical practice across care settings.
