Catastrophic Care Coordination: Managing Traumatic Brain and Spinal Cord Injuries across Workers' Compensation and Private Concierge Settings 1 hr RN and CCM FEE: Members: $0 Non-Members: $40 Abstract: Traumatic brain injuries (TBI) and spinal cord injuries (SCI) are among the most complex and resource-intensive conditions case managers encounter, often requiring lifelong coordination across multiple care settings and disciplines. This session will examine best practices for managing catastrophic injury cases within both Workers’ Compensation and Private Concierge care models, highlighting the distinct goals, challenges, and opportunities of each approach. Drawing on clinical neuroscience and case management expertise, presenters will discuss strategies for acute stabilization, rehabilitation planning, durable medical equipment procurement, home modifications, cognitive and behavioral support, community reintegration, and long-term care planning. Participants will also explore the critical role of communication and collaboration between internal (hospital-based) and external (field) case managers, with a focus on effective transitions of care, information sharing, and overcoming common system barriers.
