- Describe current federal and state policy trends that influence care coordination and transitions of care across clinical settings.
- Explain reimbursement and regulatory mechanisms that support effective care transitions, including commonly used CPT codes and emerging value-based care models.
- Discuss evolving innovations- such as artificial intelligence and new federal care coordination initiatives-and their potential impact on clinical practice and patient outcomes.
