In this presentation, the participant will be able to define the change management models related to various organizational care delivery systems to discover the potential for improvements in health equity, care, outcomes, and satisfaction. Using the Professional Science (ProSci) Awareness, Desire, Knowledge, Ability, and Reinforcement (ADKAR) model as a practical application of improving standards of case management practice.
