Stroke remains a major health concern worldwide, often leaving survivors with lasting physical impairments and significantly reduced quality of life (QoL). Addressing these challenges requires more than traditional clinical care—it demands a coordinated, patient-centered approach.

With an average modified Rankin Scale (mRS) score of 3.33 among patients—indicating considerable functional limitations—this project sought to enhance poststroke outcomes through the implementation of a comprehensive, evidence-based case management model.
Spanning from May 2022 to July 2023, the project was executed in three structured phases:
