ABSTRACT:
Military, VA, and DoD/DHA (MVD) case managers practice in one of healthcare's most dynamic and complex environments, coordinating care across multiple systems, disciplines, and communities while supporting Service Members, Veterans, and their families throughout the continuum of care. As healthcare continues to evolve, MVD case managers must be equipped with the knowledge, leadership, and collaborative strategies needed to address emerging clinical challenges, navigate complex transitions, leverage innovative technologies, and improve patient outcomes.
Mission Ready: Advancing Case Management for Military and Veteran Populations is a comprehensive educational experience designed specifically for case managers serving both active military service members, Veterans, and their families. Through eight expert-led sessions, participants will explore evidence-based practices and innovative approaches to caring for medically complex populations, strengthening transitions of care, integrating palliative and specialty rehabilitation services, addressing emerging clinical risks, leveraging artificial intelligence responsibly, and enhancing interdisciplinary collaboration across healthcare systems.
Although many sessions highlight military and Veteran-specific programs, the principles of coordinated care, patient advocacy, ethical decision-making, and interdisciplinary practice are broadly applicable to case managers across diverse healthcare settings. Participants will leave with practical tools, strategies, and resources that can be immediately incorporated into practice to improve continuity of care and optimize outcomes for Service Members, Veterans, and their families.
OBJECTIVES:
- Analyze evidence-based strategies that improve care coordination, interdisciplinary collaboration, and continuity of care for medically complex populations across military and Veteran healthcare.
- Evaluate emerging clinical practices, specialized resources, and innovative technologies—including artificial intelligence—that enhance patient-centered care, support informed decision-making, and improve health outcomes.
- Apply best practices for coordinating transitions of care, managing specialty populations, and connecting patients and families with clinical and community resources throughout the continuum of care.
- Integrate collaborative approaches that strengthen partnerships among MVD case managers, community providers, caregivers, and interdisciplinary teams to improve quality, safety, and patient outcomes.
- Develop practical strategies to address emerging challenges in military and Veteran healthcare while advancing equitable, evidence-based, and person-centered case management practice.
