CMSA's Integrated Case Management:  A Manual for Case Managers by Case Managers (2nd Edition)

CMSA is excited to announce the release of the new Integrated Case Management manual (2nd Edition).

Since 2006, the Case Management Society of America (CMSA) has been building and nurturing the Integrated Case Management Program.

The integrated model has been in practice and has been studied for over 20 years in Europe. The decision to bring this advanced case management practice model to the United States resulted in CMSA becoming a leader in the care of individuals challenged with complex health conditions.

The literature published overwhelmingly demonstrates health improvements when this patient-centered, holistic approach reduces health risks. The recent focus on complex care models in the United States has resulted in the muddling of terms. Complexity, comorbidity, and multimorbidity are often confused or used interchangeably. To focus on complex care, the correct definitions must be grasped.

KEY FEATURES OF INTEGRATED CASE MANAGEMENT:
  • Aligns with the newly revised CMSA Standards of Practice, 2022.
  • Helps learners and readers enhance their ability to work with complex patients and learn how to apply evidence-based assessments, as it fosters safe and high-quality care.
  • Teaches case managers to evaluate patients for medical and mental health barriers to coordinate appropriate integrated interventions and treatment planning.
  • ICM Tool integrates biological, psychological, social and health system assessment.
  • Supports adult, elderly, pediatric and other specialty patient populations.
