CMSA, in partnership with TPN.health, brings you a timely and practice-focused webinar designed to support case managers and allied professionals working across the care continuum. Speaker Info:
Sachie German Koufalis, LMSW, MPH, CCM, is a Behavioral Health HEDIS Manager at Healthfirst with more than 15 years of experience in behavioral health, case management, utilization management, clinical programs, and clinical quality. She also works as a medical social worker at Montefiore Medical Center, providing psychosocial assessments, discharge planning, and care coordination. Sachie serves on the Care Management Test Item Writing Committee for The Commission and the Education Committee for the Case Management Society of America.
