CMS TEAM Model What is TEAM?
The Transforming Episode Accountability Model (TEAM) is a mandatory, episode-based alternative payment model (APM) from CMS that shifts reimbursement from fee-for-service to value-based bundled payments for selected surgical procedures.

  • Start date: January 1, 2026
  • End date: December 31, 2030 (5-year model)
  • Participants: Selected acute care hospitals in designated geographic areas (CBSAs), with mandatory participation for included hospitals

Core Concept
TEAM holds hospitals financially accountable for the total cost and quality of care across a defined surgical episode, rather than paying separately for each service.

  •  Episode begins: Hospital admission or outpatient procedure
  • Episode ends: 30 days post-discharge
  • Includes: All Medicare Part A and B services related to the episode (inpatient, outpatient, physician, post-acute, etc.)
