Limited admissions and downsizing have left many older adults without the necessary care and placement, while current residents risk displacement (American Health Care Association, 2023). This issue is compounded by the increasing demand for long-term care services. One solution is the federal Money Follows the Person (MFP) program, which helps states rebalance their long-term care systems by transitioning individuals with long-term illnesses or disabilities from institutions back into the community. Successful and sustained transitions depend on factors such as planning, coordination, housing availability, and access to home and community-based services (Colello, 2021). However, a significant gap identified is the lack of resources for integrating into everyday community life. Individuals often struggle with daily tasks previously managed in institutional settings, such as getting a haircut, finding a church, or mailing a letter.
