Improving Hypertension Control in the Black and African American Population Hypertension remains a leading risk factor for cardiovascular and renal disease, with well-documented disparities in both prevalence and control across racial groups. Our Population Health team conducted an in-depth analysis of hypertension control outcomes, stratified by race, to better understand inequities within our patient population. This analysis revealed that Black and African American patients were 7% less likely to have controlled hypertension compared to other racial groups. This finding is particularly concerning given that Black and African American individuals are approximately 30% more likely to develop hypertension, underscoring a critical and persistent health inequity. Uncontrolled hypertension significantly increases the risk of adverse health outcomes, including myocardial infarction, stroke, heart failure, dementia, and chronic kidney disease—conditions that substantially diminish quality of life and contribute to increased healthcare utilization and costs. Recognizing the disproportionate burden borne by Black and African American patients, our team prioritized targeted interventions aimed at improving hypertension control within this population. Grounded in evidence demonstrating that effective blood pressure management can prevent or delay the onset of chronic disease, we implemented focused outreach and care strategies to support improved clinical outcomes.
