Improving Substance Use Disorder Outcomes Through a Scalable Health Plan–Provider Collaboration   1 hour RN, SW, and CCM  FEE:  Members / Non-Members:  $20 Tax deductible donation.   ABSTRACT:  This panel discussion explores a scalable, collaborative model between a health plan and provider organizations designed to improve outcomes for individuals living with chronic substance use disorder (SUD). Panelists will share real-world insights into the day-to-day operations of the program, highlighting how interdisciplinary roles work together to support engagement, continuity of care, and person-centered treatment planning. Moderated by the Vice President of Behavioral Health, the session features perspectives from a peer support specialist, a behavioral health field care manager, a program manager, and the program director. Attendees will gain practical strategies for integrating case management, motivational interviewing, and provider partnerships to enhance treatment engagement and program sustainability across care settings.    OBJECTIVES:  At the conclusion of this session, participants will be able to:  Describe how a strong treatment alliance supports engagement and retention for individuals living with chronic substance use disorder.  Apply integrated case management and motivational interviewing techniques to develop person-centered care plans for individuals with SUD.  Identify strategies for building collaborative relationships with facility leadership to implement and sustain onsite case management programs.
